A continuing health story

Your health story shouldn't start over
at every doctor's office.

Sofirene is being built to help your health story move with you—connecting patients, providers, and the people authorized to help care for them.

A different starting point

Healthcare is connected.
The information often isn't.

Your care may involve a primary care physician, specialists, hospitals, pharmacies, and people you trust. As you move between them, your story can become fragmented.

Sofirene is being designed around a different idea: keep the patient's health story moving with them.

One continuing story

Different roles.
Connected around the patient.

Each person sees and contributes to care differently. Sofirene is designed to preserve those differences while making it easier for the right information to move between the right people.

For patients

01

Your story, carried forward.

Patients can prepare for care over time instead of trying to remember everything at once. They can describe what they are experiencing, keep important information available, and carry their health story forward from one encounter to the next.

  • Prepare
  • Describe
  • Understand
  • Follow up

For providers

02

Better information. Better tools.

Providers can review better-prepared information, communicate with patients, coordinate care, and follow what happens after a referral, while keeping patient-reported information distinct from clinician-established information.

  • Review
  • Communicate
  • Coordinate
  • Act

Loved ones & authorized caregivers

03

Make it easier to help.

With appropriate authorization, people a patient trusts can help remember, coordinate, follow, and support care without being confused with the patient or healthcare professional.

  • Help
  • Remember
  • Coordinate
  • Support

The goal isn't to make everyone's role the same. It's to make continuity easier between them.

A better medication conversation

More than checking a box.

Medication lists are important. But knowing the name, dose, and frequency of a medication doesn't always tell the whole story.

Sofirene is being designed to let patients describe what taking a medication is actually like for them: what they've noticed, what has changed, what concerns them, and what they want their care team to know.

The usual questions

  • Are you still taking it?
  • What dose?
  • How often?

The patient's experience

Illustrative patient statements

“It helps, but I feel different after I take it.”
“I'm taking it, but not exactly the way I was before.”
“Something changed after I started it.”
“I want my doctor to know what I've been noticing.”

Referrals that keep care moving

A referral shouldn't mean starting over.

Sofirene is being designed to help carry the important parts of the patient's story forward — and help keep the people involved in their care connected along the way.

  1. Your doctor
  2. Your referral
  3. Your next provider
  4. Your continuing care

Less searching. More moving forward.

Finding the right place for a referral can mean calling offices about insurance acceptance and availability, then trying to get an appointment. Sofirene is being designed to make that transition easier.

And care shouldn't disappear after the referral.

Sofirene is being designed to help relevant information from the next part of care make its way back to the clinicians who remain involved.

Patient Consult

Sometimes you need an answer,
not another appointment.

A question can come up after the visit is over. Sofirene is being designed to help patients ask focused questions with the relevant parts of their health story already connected — so the clinician can understand what the question is about without the patient having to start from the beginning.

  1. Your question
  2. Relevant context
  3. Your care team

The question shouldn't lose its story.

Sofirene is being designed to keep a patient's focused question and relevant context together as it moves through care.

More context before responding.

A focused question with relevant information can give a clinician a clearer starting point for deciding what may help next. The clinician or practice can decide whether that is an answer, more information, the right person to speak with, or an appointment.

When care is unexpected

When the unexpected happens,
your health story should still be with you.

In an unexpected hospital or emergency-care visit, the people caring for you may not already know your history. Sofirene is being designed to help important parts of your continuing health story be available to appropriately authorized healthcare professionals when they're needed.

From your continuing health story

  • Medications
  • Allergies
  • Important health history
  • Recent care
When care is unexpectedHospital or emergency care

You shouldn't have to remember everything at the hardest moment.

Unexpected care can leave you stressed, uncomfortable, or unsure of details. Sometimes you may not be able to explain everything you normally could.

Care can change locations. Your story should keep moving.

Where appropriate and authorized, Sofirene is being designed to help relevant information from unexpected care become part of the continuing story available to the clinicians who remain involved.

Time to think

Your best questions don't always
come to you in the doctor's office.

At home, you have something often limited during an appointment: time. Sofirene is being designed to give you space to prepare and think about your care between visits.

  1. RememberSomething you meant to mention.
  2. CheckA medication, date, or detail.
  3. UnderstandA recommendation or what happens next.
  4. AskThe question that came to you later.

Before the visit

Take time to prepare.

Remember concerns, check details, review medications, and organize the questions you want to bring.

After the visit

Take time to understand.

Return to what your clinician recommended, think it through, and consider what you want to ask next.

Care doesn't only happen during the appointment.

Our approach

Technology should make healthcare
feel more connected, not more complicated.

Sofirene is being built around a simple idea: technology should help patients and the people caring for them understand each other and stay connected across care.

Your health story stays centered on you.

Sofirene is designed around you and your health story as care moves between people and places.

Your voice remains part of the story.

What you remember, notice, experience, and want to ask belongs in the conversation alongside information from your healthcare professionals.

Clinical decisions stay with clinicians.

Sofirene can help you prepare, share information, and communicate; diagnosis and treatment decisions remain with your healthcare professionals.

The story should keep its context.

As information moves through care, it should remain clear what it relates to and where it came from.

Why Sofirene

Built from experience with the problem.

Sofirene grew from more than a decade of experience building healthcare information systems across multiple organizations, and seeing how difficult it can be for a patient's information and communication to follow them from one place of care to another.

That experience led to a different question:

What if continuity could be built around the patient?
